Before your first appointment at DB2, please complete the following:
Fill out the New Patient Information Packet that was emailed to you.
Bring a list of current prescriptions and/or over-the-counter medications you are taking, including the dose and frequency.
Bring any relevant information about your medical and surgical history, as well as any x-rays or other helpful records you may have.
Be sure to arrive at least 20 minutes before your first appointment to complete any other necessary paperwork.
Please inform our office as soon as possible if you cannot make your scheduled appointment, and remember that we do not take insurance of any kind. HSA and CareCredit cards are accepted, as are all major credit cards.
