Please bring the following:

• A list of current prescription or over-the-counter medications you are taking, including the dose and frequency

• Pertinent information about your medical and surgical history. Bring any pertinent x-rays or records you may have.

Because of our Relationship Focused model, we sometimes run late as we will always ensure we are attending to our patients needs. We will make every attempt to alert you if this occurs. Likewise, please inform our office as soon as possible if you cannot make your scheduled appointment.
